Job summary

Park Ridge Living Center, part of Rochester Regional Health, seeks a nurse leader to oversee RN/LPN/CNA and clerical staff on the Long-Term Care unit. You will support the nurse manager, coordinate care plans, and help the unit hit quality, safety and operational targets while delivering strong patient care.

Qualifications include NYS RN license and 1–3 years of nursing experience, with BLS certification. The role offers evening/night shifts, weekend rotations, full benefits, and opportunities

Qualifications

• RN license in NYS is required.
• 1–3 years of nursing experience required.
• Graduate of an accredited registered nurse program.

Responsibilities

• Oversees & assures short & long term Nursing Dept. staff schedules per established guidelines.
• Facilitates units in emergency medical assessment & care of residents including interventions, transportation & other resources needed.
• Serves as a clinical resource person to staff in problem solving, assessments, special procedures & clinical skill enhancements.
• Coordinates with the Nursing Staff resident units & admissions dept. the rapid admission of residents including obtaining evaluations as indicated.
• Serves as adjunct to staff development coordinator in promoting planned in services to nursing staff as per master schedule/plan.
• Provides continuity of resident status with hospital supervisory staff, and Nursing Administration.
• Supervises the M.D.R. directing, interacting with residents, families and staff in providing safe, pleasant mealtime and assuring appropriate completion of required records.
• Serves as major facilitator in QA program for Nursing Dept. including completion of assessments of designated audits & resultant actions required.
• In absence of Nurse Manager, provides staff counseling, discipline as per policy.
• Assists with Charge Nurse duties as needed while maintaining overall supervisory responsibility.
• Participates in Nursing department and facility QI program.
• For PRLC Only: Assists with Charge Nurse duties on alternate floors on weekends while maintaining overall supervisory responsibility on weekends.
• For PRLC Only: Evaluates and makes Medicare determinations of any patients readmitted on weekends and attempts to notify families of decisions
