HealthPartners seeks a Mental Health Practitioner to join the RHSC Community Support Programs in St. Paul and Maplewood. You will provide rehabilitative mental health services, support residents, and help implement treatment plans under supervision.

Responsibilities include crisis intervention when needed, case management coordination, and maintaining detailed EMR documentation. The role requires collaboration with a clinical team and adherence to safety policies.

Qualifications

• High school diploma or equivalent.
• Master’s or graduate degree in behavioral sciences is required.
• Licensure/Registration/Certification: Medication Administration Certification or will complete required training within 90 days.
• DHS Background Study clearance required.
• Excellent communication and confidentiality skills.
• Ability to work with residents under supervision and follow treatment plans.

Responsabilités

• Provide rehabilitative mental health services as identified in the recipient's treatment plan.
• Maintain safety and welfare of program residents during assigned shifts.
• Document activities in electronic medical record notes.
• Provide crisis interventions as necessary.
• Coordinate case management as directed by the treatment plan and mental health professional.
