Qualifications

• Active Vermont RN license or eligibility for VT licensure.
• Active Vermont LPN license or eligibility for VT licensure.
• Experience in memory care, geriatrics, or long-term care settings preferred.

Responsibilities

• Provide skilled nursing care to residents with memory-related conditions.
• Assess, monitor, and document residents’ health status.
• Administer medications and treatments per physician orders.
• Respond to changes in resident condition and communicate with the care team.
• Collaborate with nursing assistants, providers, therapists, and interdisciplinary staff.
• Maintain accurate clinical documentation and promote safety, dignity, and comfort.
