Job summary

UVM Health Network- Porter Medical Center in Middlebury, Vermont, seeks an RN or LPN for a night shift on a long‑term care/assisted‑living unit. The role is full time, 36 hours per week, with rotating weekends and shift times from 6:45 PM to 7:15 AM.

The successful candidate will provide skilled nursing care, assess residents, administer medications, and document care in the EMR while collaborating with an interdisciplinary team in a compassionate, resident‑centered environment.

Qualifications

• Current Vermont RN license in good standing.
• Current Vermont LPN license in good standing.
• Strong clinical assessment and communication skills.
• Ability to work independently during overnight hours.

Responsibilities

• Provide skilled nursing care to long‑term care and assisted living residents.
• Assess resident conditions and communicate changes appropriately.
• Administer medications and treatments according to physician orders.
• Monitor resident health, safety, comfort, and overall well‑being.
• Complete accurate and timely documentation in the electronic medical record.
• Collaborate with providers, therapists, nursing assistants, and interdisciplinary team members.
• Support residents and families with compassion, professionalism, and respect.
• Serve as an advocate for resident‑centered care and quality outcomes.
