Center for Urban Community Services (CUCS) is seeking a Medication Case Manager to support residents in shelters and transitional housing, working with LPNs and medical providers to monitor medications and assist with self-administration. Hours are 4pm–12am, Monday through Friday, at our Brooklyn location.

Qualifications

• Must have a HS diploma or equivalent with 4 years of relevant work experience (CM2).
• BSW with 1–3 years relevant experience (CM3/CM4) as applicable.
• Proficient in documenting and using client management systems and internal databases.

Responsibilities

• Monitor clients self-administered medications and counsel to promote adherence.
• Maintain medication room and documentation per agency policies.
• Manage CMS and other databases; ensure timely, accurate service documentation.
