Job summary

In accordance with Vermont law, the LPN contributes to assessment, patient care, and evaluation while maintaining safety and documentation standards in a trauma-informed environment.

Qualifications

• Current, active LPN license in Vermont.
• Graduate of an approved practical nursing program (certificate or associate).
• BLS certification or ability to obtain upon hire.

Responsibilities

• Provide direct nursing care to patients undergoing detoxification or substance use treatment.
• Monitor and document vital signs, withdrawal symptoms, and overall patient condition.
• Administer medications (including detox protocols) as ordered by a licensed provider.
• Observe and report changes in patient condition to supervising RN or provider.
• Assist with medical procedures such as wound care, specimen collection, and basic treatments.
• Contribute to assessment data collection and care plan development under supervision.
• Implement nursing interventions and support continuity of care across shifts.
• Collaborate with counselors, case managers, and physicians.
• Monitor withdrawal using standardized tools and support MAT protocols.
• Provide patient education on recovery, relapse prevention, and medication adherence.
