Job summary

Masonicare at Mystic in Mystic, CT is seeking an LPN for the evening shift. This 32-hour-per-week role offers patient-centered care in assisted living/memory care units, with a focus on collaborative care planning and resident education.

Under supervision, you will oversee medication management, participate in care planning meetings, and support residents and families with compassionate nursing care. A current LPN license and CPR certification are required, with 1–2 years of relevant experience

Qualifications

• Current LPN license and CPR certification.
• 1-2 years clinical experience preferred.
• Assist with medication oversight under supervision.

Responsibilities

• Contributes to resident care plans by data collection and care planning meetings.
• Communicates with residents, families, and care team to meet needs.
• Provides education and counseling on AL procedures.
• Documents information in the client service record.
• Conducts supervisory visits of care assistants as required.
• Participates in educational programs and workshops to improve skills.
• Oversees medication management with supervision, including ordering and preparation.
• Performs additional duties as assigned.
