Job summary

Three Meadows Post Acute in Perrysburg, OH is hiring a 2nd shift LPN to lead the evening unit and provide hands-on nursing care to residents. You’ll supervise CNAs, monitor conditions, and coordinate with physicians, families, and the care plan team.

Ideal candidates are Ohio-licensed LPNs with CPR, leadership skills, and compassionate patient care for older adults. The role offers a strong team environment and opportunities to influence quality of care.

Qualifications

• Graduate of an approved LPN program or equivalent.
• Current, unencumbered Ohio LPN license in good standing.
• Current CPR certification and strong leadership skills.

Responsibilities

• Provide hands-on nursing care including meds, treatments and wound care.
• Lead the evening shift by supervising CNAs and daily rounds.
• Coordinate with residents, families and physicians on orders and updates.
• Maintain and monitor care plans with the care plan team.
• Assist new residents with admission, orientation and discharge planning.
• Promote a safe, infection-free environment and model safety practices.
