Job summary

Pathways Center for Behavioral and Developmental Growth in Georgia is hiring a Licensed Practical Nurse for a 2nd shift Crisis Home role in Greenville, GA. You will work with the treatment team to provide direct care per the individual treatment plan and support individuals with IDD in a crisis context.

Responsibilities include taking vital signs, administering medications, assisting with daily living activities, documenting changes, and coordinating transportation to appointments.

Qualifications

• Vocational/Technical degree from an accredited college.
• Current Georgia license to practice as a Licensed Practical Nurse.
• One year of nursing experience.

Responsibilities

• Work with the treatment team to provide direct care to individuals residing in Crisis Home based upon the individual treatment plan.
• Measures and records clients' vital signs, such as height, weight, temperature, blood pressure, pulse and respiration.
• Care includes providing medical treatments, administering medication and giving injections.
• Helps individuals with bathing, dressing, and personal hygiene, moving in bed, and standing and walking.
• Observes individuals in Crisis Home and documents changes in client conditions or adverse reactions.
• Notifies appropriate team members and discusses necessary actions.
• Participates in developing and reviewing treatment plans; transports or arranges transportation to appointments.
