Job summary

Caledonia Senior Living & Memory Care in North Riverside, Illinois is seeking an Evening Shift Licensed Practical Nurse (LPN) to provide compassionate, high-quality resident care on our evening unit. You will administer medications, support daily living activities, and collaborate with the interdisciplinary team to maintain safety and comfort for residents.

The role requires an Illinois LPN license, BLS, strong teamwork, and proficient use of electronic medical records.

Qualifications

• Current LPN license in Illinois.
• BLS certification on hand.
• Ability to work as part of a care team and communicate effectively.
• Strong observation and assessment skills.
• Ability to lift and assist residents as needed.
• Familiarity with electronic medical records.

Responsibilities

• Provide direct care to residents, including administering medications, wound care, and personal care
• Develop and implement individualized care plans in collaboration with the interdisciplinary care team
• Monitor and report changes in residents' condition to the nursing team and other appropriate staff members
• Collaborate with other departments, such as social services and activities, to ensure comprehensive care for residents
• Participate in quality improvement initiatives and maintain accurate and up-to-date records
• Demonstrate a commitment to excellence in patient care and a passion for working with seniors
