Qualifications

Responsibilities

• Develop and implement individualized care plans with the interdisciplinary care team
• Monitor and report changes in residents' condition to the nursing team
• Collaborate with other departments to ensure comprehensive care
• Participate in quality improvement initiatives and maintain accurate records
• Demonstrate a commitment to excellence in patient care and working with seniors
