Job summary

Signature HealthCARE of Elizabethtown is hiring a full-time LPN for the 3pm–11pm (8-hour) evening shift on the treatment floor. The role involves direct nursing care, supervision of aides, and admissions support in a long-term care setting.

Qualifications

• Active LPN/LVN license in state of practice.
• Minimum 1 year clinical nursing experience; LTC experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong observation, medication administration, and documentation skills.
• Familiarity with Federal and State long-term care regulations.
• Excellent verbal and written communication skills.

Responsibilities

• Provide direct nursing care to residents, including med administration and wound care.
• Assist residents with ADLs ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides per policy.
• Monitor residents’ status and notify physician of changes.
• Develop and review individualized care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in QA activities and staff education as needed.
• Perform other duties as assigned by Director of Nursing.
