Signature HealthCARE of Elizabethtown is hiring an LPN/LVN to provide direct nursing care on the treatment floor and assist with admissions paperwork. The role features an 8-hour shift, evenings from 3pm to 11pm, and full-time hours with no weekends.

Ideal candidates hold an active LPN/LVN license in the state, with preferred supervisory experience and familiarity with EHR systems. A strong focus on patient safety, documentation, and teamwork is required.

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care preferred.
• Experience in supervisory/charge nurse role preferred.
• Strong observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify physician and responsible party of changes.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy.
• Coordinate resident admissions, transfers, and discharges per procedures.
• Participate in quality assurance, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
