Qualifications

• Active Virginia LPN license in good standing.
• CPR/AED and First Aid certification (or ability to obtain).
• Strong communication and accurate documentation skills.
• Knowledge of medication administration and nursing practices.
• Ability to work with Direct Support Professionals and nursing staff.

Responsabilités

• Provide direct nursing care and implement physician orders and care plans.
• Monitor vital signs and health indicators; report changes promptly.
• Review MARs and treatment documentation for accuracy.
• Coordinate with pharmacies for medications and treatments.
• Educate and reinforce nursing care protocols with staff.
• Assist with medical appointments and follow-up care.
• Maintain nursing documentation per policies and Medicaid standards.
• Promote infection control and person-centered care.
• Collaborate with interdisciplinary team to ensure continuity of care.
• Perform additional duties as assigned within LPN scope.
