Qualifications

• Currently licensed as LPN in Pennsylvania.
• 1-2 years’ experience working as an LPN.
• CPR/First Aid certification is required.
• Must be 21 years of age or older.
• Valid driver’s license is required.
• Understanding of addictions and EMR/charting knowledge.

Responsibilities

• Reports observations and supports medical treatment plans under RN oversight.
• Serve as a resource to treatment team members.
• Enter and review medical records and document patient histories accurately.
• Prepare patients for examinations, collect vitals, and handle labs per policy.
• Administer medications and manage controlled substances with accuracy.
• Schedule patient appointments and coordinate with the physician.
• Attend mandatory trainings and participate in multidisciplinary meetings.
• Maintain clean, organized work environment and quality control logs.

Education

Pennsylvania LPN license

1-2 years LPN experience

CPR/First Aid certification

Driver's license

21+ years old
