Qualifications
- BS or PharmD from an accredited school of pharmacy
- Current pharmacist license in the state of practice
- Collaborate with physicians, nurses, and pharmacy staff to optimize therapy
- Provide drug information and patient counseling as needed
- Participate in medication use evaluations and safety initiatives
Responsibilities
- Review patient records to assess appropriateness of medication therapy
- Evaluate patient conditions to ensure proper treatment
- Develop and implement safe, effective medication plans
- Monitor drug therapy and intervene when appropriate
- Assess outcomes and adjust treatments as needed
- Provide counseling to patients on proper medication use
- Collaborate with physicians and nurses on medication selection and dosing
- Identify and prevent adverse drug events and interactions
