Job summary

Chandler Hall is seeking an experienced Registered Nurse to provide evening triage, urgent hospice visits, and admissions during the 3:00 p.m.–11:30 p.m. shift.

The RN will respond to calls, assess symptoms, provide guidance, and perform admissions while traveling across Bucks County, Montgomery County, and Northeast Philadelphia. Qualifications

• Active Pennsylvania RN license.
• Hospice nursing experience preferred.
• Experience with triage and admissions processes.
• Strong communication and care coordination skills.
• Ability to document care accurately.

Responsibilities

• Receive and respond to evening calls from hospice patients, caregivers, facilities, and healthcare providers.
• Assess symptoms and changes in condition to determine clinical response.
• Provide telephone support, education, and symptom-management guidance.
• Complete in-person visits for urgent symptoms and hospice admissions.
• Coordinate medications, equipment, and services for the shift.
• Document calls, assessments, and orders accurately and promptly.
• Provide clear handoffs to the next shift and care team.
• Travel throughout Bucks County, Montgomery County, and Northeast Philadelphia.
