Job summary

Chandler Hall in Newtown, PA is seeking an experienced RN for the Hospice Evening On-Call, Triage and Admissions role. The 3:00 p.m.–11:30 p.m. shift involves triage visits and admissions across Bucks County, Montgomery County, and Northeast Philadelphia.

The RN will respond to calls from patients and families, assess symptoms, provide guidance, perform in-person visits when needed, and coordinate medications and equipment. Travel required, and adherence to hospice regulations is essential.

Qualifications

• Current Registered Nurse license in Pennsylvania.
• Hospice nursing experience is preferred.
• Hospice admissions and telephone triage experience preferred.
• CHPN certification preferred.
• Strong knowledge of end-of-life care and symptom management.
• Ability to travel throughout Bucks County and surrounding areas.

Responsibilities

• Receive and respond to evening calls from hospice patients, families, facilities, and providers.
• Assess symptoms and changes to determine clinical response.
• Provide telephone guidance and symptom-management education.
• Complete in-person visits for urgent needs and hospice admissions.
• Collaborate with hospice physicians and other providers to obtain orders.
• Coordinate medications, equipment, and services for the evening shift.
• Document calls, assessments, interventions, visits, and orders accurately.
• Handoff admissions or changes to the next team with clear details.
• Support patients and families with calm, compassionate guidance.
• Follow Chandler Hall policies and hospice regulations.
