Job summary

Hospice & Community Care in Lancaster, PA, is seeking an RN for Admissions/Care Transitions. This PRN role supports hospital-based patient transitions across Lancaster and surrounding counties.

Orientation is 5 weeks full time, 8 a.m.–5 p.m., with 8- or 12-hour shifts. Responsibilities include conducting admission visits, comprehensive nursing assessments, documenting in hospital and hospice EMR, coordinating with physicians and care teams, and assisting discharge planning and referrals to

Qualifications

  • Graduate of an accredited school of registered nursing.
  • Current Pennsylvania nursing license.

Responsibilities

  • Provides accurate and timely information regarding hospice services to any inquirer.
  • Completes admission visits for routine, general inpatient (GIP) and Hospice Response Team (HRT) patients.
  • Performs RN comprehensive assessments and focused skilled nursing visits on GIP/HRT per discipline frequencies.
  • Follows up on hospice waiting patients to assure a smooth hospital discharge.
  • Documents in the hospital EHR and hospice EMR accurately and concisely at the time the service is provided.
  • Obtains accurate patient‑related data including initial needs assessment, medical history, insurance information and resources available to patient and caregiver.
  • Immediately communicates assessment information and proposed plan and level of care to the attending physician, hospital care/case manager, and access team, requesting additional information, orders, or follow‑up as needed.
  • Collaborates with community residential facility staff to develop an effective and safe discharge plan.
  • Provides ongoing communication and updates with organizational staff, hospice physicians, patients, families, hospital staff, community palliative care providers, community partners and admissions planners.
  • Assists with hospital transition/discharge planning process, working with case/care management departments, nursing staff and caregivers to facilitate admission to hospice services.
  • Completes hospice enrollment including informed consent as appropriate.
  • Assists with outreach to referral sources to coordinate care and facilitate the referral process.
  • Anticipates, assesses, and responds to pain and other symptoms of physical, emotional, and spiritual suffering.
  • Demonstrates thorough knowledge and application of current palliative care protocols for symptom management.
  • Provides direct nursing care and interventions as prescribed by the physician and/or symptom management orders.
  • Works independently and effectively throughout all care settings as required.
  • Understands reimbursement and payer source implications to the plan of care and utilizes resources wisely and effectively.
  • Assists with the orientation of new staff and visiting professionals.

Education

Graduate of an accredited school of registered nursing
Current Pennsylvania nursing license

RN - Admissions/Care Transitions

Hospital Based. PRN as needed. Option: 8 or 12‑hour shifts. Mostly working in a hospital setting, covering Lancaster, York and surrounding counties. Orientation: 5 weeks full time, 8 a.m. – 5 p.m.

PRN Requirements
  • 2 shifts per month
  • 1 weekend per quarter
  • 1 holiday every 2 years

Qualifications
  • Graduate of an accredited school of registered nursing.
  • Maintains current Pennsylvania license.
  • One year of nursing experience as a registered nurse within the last four years required.
