VNA Hospice NWI in Valparaiso, IN is seeking a compassionate Registered Nurse (RN) for our 6-bed hospice inpatient care center on the evening shift.

You will provide expert symptom management, coordinate with a multidisciplinary team, and support patients and families through end-of-life care in a peaceful unit.

Qualifikationen

• Current Indiana RN license in good standing.
• Graduate of an accredited nursing program.
• Current CPR certification.
• Ability to work independently and collaboratively within an interdisciplinary team.
• Excellent communication and critical thinking skills.

Aufgaben

• Assess, plan, implement, and evaluate individualized patient care plans.
• Manage pain and symptoms using hospice and palliative care best practices.
• Administer medications, treatments, and therapies per physician orders.
• Monitor patient condition and communicate changes to the interprofessional team.
• Provide education and emotional support to patients and families.
• Coordinate patient care with physicians and team members.
• Maintain accurate documentation in the EMR.
• Advocate for patient comfort, dignity, and end-of-life wishes.
• Participate in team meetings and quality improvement initiatives.
