VNA Hospice of Northwest Indiana is seeking a compassionate Registered Nurse (RN) to join our 6-bed Hospice Inpatient Care Center in Valparaiso, IN. This full-time evening shift provides skilled nursing care to patients at end-of-life, with rotation every other weekend.

The RN collaborates with an interdisciplinary team to ensure comfort, dignity, and quality of life, managing symptoms and supporting families through the hospice journey.

Qualificações

• Current Indiana Registered Nurse (RN) license in good standing.
• Graduate of an accredited nursing program.
• Current CPR certification.
• Ability to work independently and collaboratively within an interdisciplinary team.
• Excellent communication and critical thinking skills.

Responsabilidades

• Assess, plan, implement, and evaluate individualized patient care plans.
• Manage pain and symptoms utilizing hospice and palliative care best practices.
• Administer medications, treatments, and therapies in accordance with physician orders.
• Monitor patient condition and communicate changes to the hospice interdisciplinary team.
• Provide education and emotional support to patients and their families.
• Collaborate with physicians and team members to coordinate patient care.
• Maintain accurate and timely documentation in the electronic medical record.
• Advocate for patient comfort, dignity, and end-of-life wishes.
• Participate in team meetings, care conferences, and quality improvement initiatives.
• Provide compassionate support to families throughout the hospice journey.
