Job summary

Blue Ridge Care is seeking a compassionate Registered Nurse (RN) for evening hospice care on a full-time basis. This role involves 8- or 10-hour evening shifts, every other weekend, and travel across the service area with a $10,000 sign-on bonus.

You will respond to calls, provide skilled nursing visits, educate families, and collaborate with the interdisciplinary team to ensure comfort and dignity at the end of life. BSN preferred; Virginia RN license required; CPR and CHPN preferred.

Qualifications

• Current Virginia RN license.
• Graduate of an accredited nursing program; BSN preferred.
• Two or more years hospice/home health/ICU or related nursing experience preferred.
• CPR certification.
• CHPN certification preferred.
• Knowledge of Medicare/Medicaid hospice regulations.
• Proficiency with electronic medical records.
• Ability to travel throughout Blue Ridge Care territory.

Responsibilities

• Respond to emergent calls from patients and families during evening hours.
• Provide visits to address urgent concerns, symptom changes, or end-of-life needs.
• Deliver skilled nursing care in accordance with patient goals and physician orders.
• Assess patient conditions and provide clinical oversight when LPN support is involved.
• Develop, update, and implement individualized plans of care.
• Administer medications and treatments as prescribed.
• Support symptom management and patient comfort.
• Educate patients and families on disease progression, symptom management, and care techniques.
• Provide emotional support and reassurance during difficult situations.
• Communicate significant changes in condition to physicians and obtain necessary orders.
• Complete timely and accurate clinical documentation.
• Coordinate equipment, supplies, and services needed to support quality care in the home.
• Collaborate with the interdisciplinary team to ensure continuity of care.
• Assist with admissions, crisis visits, death visits, and other after-hours hospice needs as appropriate.
• Participate in continuing education, quality improvement activities, and team meetings.
