Job summary

Ovation Hospice is seeking an outstanding Hospice Aide to join our team in Utah. The role provides personal care under the direction of a registered nurse, helping patients stay comfortable and cared for in a supportive environment.

Qualifications

• Meet Utah state training requirements.
• One year experience as CNA, hospice aide or home health aide.
• At least 18 years old.
• Must be a licensed driver with an insured automobile in good working order.
• The ability to communicate well, both verbally and in writing.
• Ability to work as a team and independently.

Responsibilities

• Document care provided according to organization policy.
• Report deviations from the Plan of Care to the Case Manager.
• Use DurableMedical Equipment safely.
• Take vital signs on Hospice Aide Plan of Care.
• Participate in QAPI activities and IDG meetings as appropriate.
• Provide proper care and observation of patient skin to prevent tissue breakdown.
• Assist with ambulation, transfer, and range of motion as documented.
• Provide respite for patients’ family/caregiver on-site as appropriate.
