Job summary

Ovation Primary Care & Hospice is seeking an outstanding Hospice Aide to join our team in West Jordan, Utah. The role involves providing compassionate personal care under RN direction and delivering patient-centered support in a Hospice setting.

Qualifications

• Meet training requirements by the state of Utah.
• One year experience as a CNA, hospice aide or home health aide.
• At least 18 years old.
• Must be a licensed driver with an insured automobile in good working order.
• The ability to communicate well, both verbally and in writing.
• Ability to work as a team and also independently.

Responsibilities

• Document care provided according to organization policy.
• Report any deviations from the Plan of Care to the Case Manager.
• Use DurableMedical Equipment safely.
• Take vital signs when on Hospice Aide Plan of Care.
• Participate in QAPI activities.
• Participate in IDG meeting, as appropriate.
• Provide respite for patients family/caregiver when on-site.
