Cadre Family Companies is seeking a Lavender Hour Aide (CNA) to provide compassionate hospice care during the late afternoon and evening, typically 4:00 PM–9:00 PM. You will support patients and families, help with personal care, and promote comfort and dignity in the home or care setting.

You will observe patient changes, report to the hospice team, and collaborate with the RN case manager and on-call nurse.

Qualifications

• Current CNA/Hospice Aide certification as required by state and federal regulations.
• Meets Medicare and organizational requirements for Hospice Aide services.
• Previous hospice/home health experience preferred.
• Ability to travel within the assigned service area.
• Comfortable working independently in patient homes and care facilities.

Responsabilités

• Provide personal care per the patient's plan of care and Hospice Aide Care Plan.
• Assist with bathing, grooming, toileting, incontinence care, repositioning, dressing and other ADLs.
• Support patients with evening routines and settling for bed.
• Offer skin care and comfort-focused interventions.
• Observe changes and promptly report to the hospice team.
• Complete accurate documentation of all care provided.
• Collaborate with RN case manager, on-call nurse, and interdisciplinary team.
