Job summary

Cadre Family Companies in Ohio is seeking a Lavender Hour Aide (CNA) to provide compassionate, hands-on hospice care during the late afternoon and evening (4:00 PM–9:00 PM). You will support patients and families with comfort, dignity, and calm as part of the hospice team.

You will observe changes in condition, communicate with the RN and case manager, and help patients transition into their evening routines within their homes or care facilities.

Qualifications

• Current CNA/Hospice Aide certification as required by state and federal regulations.
• Meets all Medicare and organizational requirements for Hospice Aide services.
• Previous hospice, home health, LTC, or end-of-life care experience preferred.
• Demonstrated ability to provide compassionate personal care with strong observation and communication skills.
• Comfortable working independently in patient homes and care facilities.
• Reliable transportation and ability to travel throughout the assigned service area.
• Ability to meet the physical requirements of the position.
• Dependable and comfortable working during late afternoon and evening hours.

Responsibilities

• Provide personal care per patient care plan and Hospice Aide Care Plan.
• Assist with bathing, grooming, toileting, incontinence care, dressing, and ADLs.
• Prepare patients for bed and settle them for the evening.
• Maintain skin care and other comfort-focused interventions.
• Respect patient dignity, privacy, preferences, and routines.
• Observe changes and communicate with the hospice team.
