Job summary

The Christ Hospital Home Health is seeking an RN On-Call to support after-hours nursing care in patients' homes in the Cincinnati area. You will respond to after-hours patient needs, provide one-on-one care, and perform chart reviews from home when no urgent after-hours needs remain.

Responsibilities include developing care plans, collaborating with the interdisciplinary team, and ensuring documentation per Medicare guidelines. Valid RN license required, home health experience preferred.

Qualifications

• RN with at least one year of experience in an acute care setting.
• Home care experience preferred.
• Excellent communication with patients, families and coworkers.
• Ability to work autonomously and manage time effectively.

Responsibilities

• Observe and monitor patient conditions and develop an individualized care plan.
• Administer medication as prescribed and help decrease rehospitalizations by prioritizing high-risk patients.
• Oversee and supervise total care of patient provided by nurse aides and LPNs.
• Collaborate with the interdisciplinary team and physicians to ensure optimal care.
• Complete SOC/ROC/Recert and subsequent documentation within 24 hours.
• Educate the patient and family on the disease process using teach-back methods.
