Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver's license with ability to transport individuals in agency vehicles.
• Ability to complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including occasional stay beyond a scheduled shift.

Responsibilities

• Providing support with daily living activities such as grooming, hygiene, and dressing.
• Assisting with continence care as needed.
• Encouraging independence and skill development.
• Monitoring health-related information and documenting health data in EHR.
• Administering medications and maintaining medication records as trained.
• Transporting individuals to appointments and community activities using agency vehicles.
• Preparing meals and supporting mealtime according to dietary needs.
• Assisting with mobility and transfers using approved techniques and equipment.
• Maintaining documentation and compliance with regulations and policies.
• Responding to emergencies and applying first aid when required.
