Job summary

Pathfinder in Plymouth, Indiana is seeking a Direct Support Professional (DSP) – Evening Floater. This full-time role provides essential daily living support, health care assistance, and community engagement for individuals with disabilities, with evening shifts 2 pm–12 am and starting pay $16.00 per hour plus differential pay.

You will administer medications, document care, uphold privacy, and collaborate with team to develop individualized support plans.

Qualifications

• Minimum high school diploma or GED.
• Able to work evenings and flexible schedule.
• Valid driver’s license and insured vehicle.
• Ability to lift up to 50 pounds and perform required physical activities.
• Clear communication and documentation skills.

Responsibilities

• Provide daily living support including personal care, meal prep, and planning to promote independence.
• Foster community engagement and meaningful connections for individuals.
• Administer medications, monitor health, and assist with medical appointments.
• Advocate for rights and choice using Person-Centered Thinking approaches.
• Develop and implement individualized support plans aligned with goals.
• Maintain accurate documentation and protect privacy.
• Collaborate with team and involve individuals in decisions and learning.
