Job summary

Sevita is seeking a Direct Support Professional to assist individuals with intellectual and developmental disabilities in community-based settings. The role supports 4–6 individuals in residential programs or up to 10 in periodic programs, with duties spanning socialization, daily living activities, and health care tasks.

The position requires a high school diploma (or equivalent), 18+ years old, basic computer skills, and may involve driving.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Directly support individuals per service plans, including socialization, health maintenance, medication administration, and skill/behavioral development.
• Assist with daily living activities: personal care, housekeeping, recreation, transportation, community integration, shopping, and citizenship.
• Maintain records and confidentiality; document progress and review logs for service plan changes.
• Build professional relationships with individuals, families, guardians and case managers.
• Report abuse or neglect and incidents per Sevita policy and external requirements.
• Respect rights, safety practices, and universal precautions.
