• Valid driver's license (Class C or D).

• Ability to support social skills and independence in home settings.

Responsibilities

• Provide one-on-one care based on approved PASS plan.
• Assist with activities of daily living as needed.
• Support independence, decision-making, and life skills.
• Create opportunities for socialization at home and in the community.
• Document progress, submit timesheets and required reports.
• Transport individuals to activities and appointments as needed.
• Maintain confidentiality and a safe, respectful environment.
