Job summary

Qualifications

• Must meet all applicable state and federal requirements for this position.
• Prefer 6 months or more of experience in a home care, assisted living, or long-term care facility.

Responsibilities

• Provide physical support to assist residents with daily living activities including transfers and vital signs.
• Observe and document residents' health, behavior, and condition; report changes to the nurse.
• Respond promptly to resident call signals and ensure resident needs are addressed.
• Promote safety and infection control; maintain a clean, safe environment.
• Communicate with nurses regarding resident conditions and concerns.
• Complete required documentation in a timely manner.
• Provide companionship, comfort and support to residents.
• Meet federal and state in-service requirements.
• Perform other duties as assigned.
