Job summary

Bend Transitional Care in Bend, Oregon, is seeking a dedicated Full-Time Certified Nursing Assistant (CNA I) to join our nursing team. The CNA will assist with activities of daily living, mobility, vital signs, and meals, while documenting and communicating changes to the team.

Qualifications

• High school diploma or equivalent required.
• CNA Oregon certification and CPR certificate required.
• Six months of long-term care experience preferred.

Responsibilities

• Provide assistance with activities of daily living (bathing, dressing, grooming, personal hygiene).
• Assist residents with mobility, transfers, and ROM exercises.
• Take and document vital signs, intake, and outputs as directed.
• Assist residents during meals and support nutritional needs.
• Observe residents and promptly communicate changes in condition to the nursing team.
• Maintain accurate and timely documentation.
• Build positive, respectful relationships with residents, families, and team members.
• Collaborate with the nursing team to deliver exceptional resident care.
