Job summary

Fathers & Families Support Center seeks an Evening Case Manager to assess and assist fathers in its programs, creating individualized self-sufficiency plans and promoting responsible fatherhood.

You will conduct social assessments, provide advocacy, and support groups while maintaining thorough documentation and coordinating with the team. Occasional travel and evenings/weekends are required.

Qualifications

  • Minimum B.S. in Human Services or related; MSW preferred.
  • Experience with at-risk populations and group/family services.
  • Valid Missouri driver’s license and reliable vehicle.

Responsibilities

  • Conduct social assessments and develop individualized self-sufficiency plans.
  • Assist clients in overcoming barriers through intervention, advocacy, and referrals.
  • Facilitate peer and family support groups and coordinate activities.
  • Maintain client records and document progress to track outcomes.
  • Travel to multiple locations and participate in after-hours events as needed.
  • Engage graduates for up to one year post-graduation to monitor goals.

Fathers & Families Support Center

Position Title: Evening Case Manager

Reports To: Director, Coordinator

Job Status: Full-Time / Non-Exempt

Direct Reports: N/A

Job Description: Responsible for assessing and assisting fathers participating in the Fathers & Families Support Center programs. Develop individual self-sufficiency plans which will help fathers become emotionally involved and financially responsible for their children.

Duties, Functions and Responsibilities
  • Rotating Schedule. Evening hours while classes are in session / Day hours when classes are not in session.
  • Completes a social assessment, which includes personal interviews as well as collection of secondary information to complete an individualized plan for self-sufficiency and responsible fatherhood.
  • Assists client in overcoming barriers through intervention, advocacy, and referrals.
  • With the support of the individual, identifies achievable goals, barriers, provides personal support and counsel to participants achieving identified goals.
  • Facilitates peer and family support groups, including the development and selection of topics and programs and securing outside guest speakers. Completes drug screening, police, and abuse records check.
  • Assists in administering the FFSC fatherhood curriculum and expands fatherhood knowledge in the FFSC classroom, the Father’s Rap session and after-school programs. Travel to multiple locations.
  • On-going case management including maintaining weekly contact with graduates for at least one year after graduation to monitor achievement of program goals.
  • Documents individual records and forms, as necessary, to provide accurate and timely tracking.
  • Develops and maintains network of community resources and social support systems, as necessary, to assist individuals and their children/families to move to self-sufficiency.
  • Review and possess a working knowledge of all programs, forms, and grants related to your duties.
  • Must be able to work flexible schedule, some evenings, and required 6 Saturday events per year.
  • Mandatory to attend all staff and department meetings, client graduations, and bonding events.
Knowledge, Skills and Abilities
  • Must have effective verbal and written communications and interpersonal skills with ability to multi-task.
  • Must be able to utilize agency and community resources to meet goals. Familiar with MS Office Products.
  • Must be able to organize and present information as well as work independently.
  • Must have strong case management and documentation skills.
  • Must have the ability to identify problematic situations to be proactive and deescalate issues as they arise.
  • Must understand and practice the NASW code of ethics.
Educational and Experience Requirements
  • Minimum requirement is a B.S. in Human Services or related; or BSW, MSW preferred.
  • Must have experience in working with at-risk populations and prior work with group facilitation, individual and family services. Must possess a valid Missouri driver’s license and have a reliable, insured vehicle.
