Community Horizons in Wisconsin is seeking compassionate in-home caregivers to provide one-on-one care for a young female member in her home setting. The role runs from about 4pm to 9pm with a two-week rotating schedule, offering flexible evening hours.
The caregiver will provide full-care assistance, including transfers with a Hoyer lift, help with showers, dressing, and daily activities, and will support meals, laundry, and light housekeeping while keeping the patient safe and engaged.
Are you tired of facility burn out? Community Horizons is assisting in finding caregivers to come in home to provide care one on one. This member is a young female member in her home setting. This position is ideal for someone who enjoys building meaningful connections and providing hands-on support with daily care needs. Requires full-care assistance, including use of a Hoyer lift, help with showers, dressing, brief changes, transfers, and other daily living activities. Care is provided in a comfortable home environment with support from family members. The member enjoys puzzles, scrabble, horses, and spending time visiting and engaging in activities. We are looking for someone who is kind, attentive, personable, and able to create a positive and respectful environment. Hours will be around 4pm-9pm and are set in a two-week rotating schedule. Provide patients with help moving in and out of beds, baths, wheelchairs, or automobiles and with dressing and grooming. Bathe patients. Care for patients by changing bed linens, washing and ironing laundry, cleaning, or assisting with their personal care. Entertain, converse with, or read aloud to patients to keep them mentally healthy and alert. Plan, purchase, prepare, or serve meals to patients or other family members, according to prescribed diets.
