Responsibilities

  • Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
  • Monitor patient health by checking vital signs and observing for any changes in condition
  • Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
  • Support medication administration following care plans and HIPAA compliance standards
  • Maintain a clean and safe environment through regular cleaning, laundry, and organization tasks
  • Use specialized equipment such as Hoyer lifts or resident lifts to ensure safe transfers and mobility assistance
  • Document care activities accurately, including patient observations, medication logs, and incident reports
  • Provide emotional support and companionship to promote mental well-being and reduce feelings of isolation

Qualifications

  • Prior experience in home health, assisted living, nursing home, or group home settings is highly preferred
  • Knowledge of patient observation techniques, dementia care, Alzheimer’s care, memory care, and hospice care practices
  • Certification in CPR and First Aid is required; additional training in HIPAA regulations is advantageous
  • Ability to work independently while following detailed care plans and protocols
  • Strong communication skills to collaborate effectively with team members and families
  • Physical ability to perform heavy lifting and assist with resident transfers using equipment like Hoyer lifts
  • Experience working with individuals with developmental disabilities, autism spectrum disorder, or other special needs is a plus
