Job summary

AdventHealth Kansas City Metro is seeking a Behavioral Health Social Worker/Therapist for part-time evening shifts. You will conduct psychosocial assessments, develop treatment plans, and provide individual, family, and group therapy within a multidisciplinary team.

The role emphasizes compassionate, evidence-based care, discharge planning, and coordination with physicians and care teams to support positive patient outcomes. A strong commitment to whole-person care is essential.

Qualifications

• Master's Degree in Social Work, Psychology, Counseling, or a related behavioral health field.
• Two years of relevant behavioral health experience — Required.
• LBSW licensure (Licensed Bachelor Social Worker)
• Basic Life Support (BLS) preferred

Responsibilities

• Conduct comprehensive behavioral health assessments, including psychosocial evaluations and risk assessments.
• Develop individualized treatment plans with measurable therapeutic goals based on patient needs.
• Provide individual, family, and group therapy utilizing evidence-based therapeutic interventions.
• Evaluate patient progress and modify treatment plans based on ongoing clinical assessments.
• Document assessments, treatment interventions, patient progress, and outcomes accurately while maintaining patient confidentiality.
• Collaborate with physicians, nurses, case managers, and interdisciplinary treatment team members to develop and coordinate patient care plans.
• Participate in multidisciplinary treatment team meetings by providing clinical recommendations and patient progress updates.
• Develop comprehensive discharge plans in collaboration with patients, families, and the interdisciplinary team, ensuring outpatient appointments and resources are arranged.
• Coordinate referrals to internal services and community behavioral health resources to promote continuity of care.
• Identify and intervene in cases involving suspected abuse or neglect with sensitivity to age-specific concerns.
• Maintain therapeutic relationships and promote recovery, resilience, and emotional well-being.
• Support departmental quality initiatives and participate in process improvement activities.
• Perform other duties as assigned
