Job summary

Brooke Glen Behavioral Hospital in Fort Washington, PA, is seeking an Assistant Assessment and Referral Director to oversee evening shift operations and assist in admissions coordination. You will lead patient assessments, document findings, and liaise with external reviewers.

The role requires a Master's in social work or related field, 2+ years clinical psychiatric experience with at least 1 year in management, and CPR/CPI certification within 90 days.

Qualifications

• Master's Degree in Social Work, mental health/behavioral sciences, or related field.
• Minimum of two years of direct clinical practice in a psychiatric/mental health setting.
• At least one year of management experience.
• Experience in patient assessment, referral, treatment planning and external review communications.
• CPR and CPI completion within 90 days; knowledge of Joint Commission, OSHA and regulatory requirements.
• Basic computer skills.

Responsibilities

• Directs the response to requests for services by phone and in person, including initial response, assessment and referral process, level of care designation, initiation into services, and follow-up.
• Completes a patient interview according to policy.
• Document patient information.
• Review the effectiveness of the initial interface with external reviewers.
• Maintain active involvement and awareness of all patient admissions, discharges and transfers.
• Share on-call responsibilities with the Director of Admissions.
• Provide initial evaluation and initiate the assessment, documenting patient symptoms/functioning/needs.
• Staff each assessment with appropriate physician, documenting recommendations and Axis I-V diagnoses when appropriate.
• In the absence of the Director, facilitate daily Access meeting and delegate responsibilities as needed.
