Qualifications

• Pharmacist licensure in the relevant state or eligible to start within 30 days.
• Bachelor's Degree in Pharmacy required; PharmD preferred.
• FPGEC certification or equivalent required; BPS certification may be role-specific.
• Postgraduate residency or equivalent clinical hospital pharmacy experience preferred.

Responsibilities

• Cross cover adult acute care areas as needed.
• Provide pharmaceutical care services aligned with best practices.
• Monitor patients and review drug therapy to prevent issues.
• Collaborate with multidisciplinary teams to deliver patient-centered care.
• Provide discharge medication review and counseling.
• Oversee pharmacy operations and mentor staff.
