Job summary

Advocate Health is seeking a Clinical Pharmacist, Acute Care (PRN/Part-time) for the Winston Campus in Wake Forest, NC. Responsibilities include overseeing medication therapy, educating patients, and collaborating with the healthcare team to ensure safe, effective pharmacotherapy.

The role requires Pharm.D. or B.S. in Pharmacy, NC licensure, and immunization certification. Prior residency or board certification is preferred. Flexible scheduling and opportunities for mentorship are available.

Qualifications

• Graduate of an accredited School of Pharmacy with a B.S. in Pharmacy or Doctor of Pharmacy (Pharm.D.) is required.
• Current state pharmacist license is required in good standing through North Carolina Board of Pharmacy.
• Immunization and non-vaccine injectable certification required based on area of practice.
• Credentialing and privileging required based on area of practice.

Responsibilities

• Oversee pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens.
• Provide counseling to patients and caregivers on medication therapy, adverse effects, compliance, and administration.
• Designs, monitors, and evaluates patient-specific pharmacotherapeutic regimens and documents per policy.
• Reviews and interprets medication orders and prescriptions accurately.
• Participates in medical emergency management when applicable.
• Supports medication safety and quality improvement activities within the department.
• Collaborates with the healthcare team to manage formulary adherence and medication-use processes.
• Ensures compliance with organizational policies and state/federal regulations.
• Serves as a preceptor for pharmacy students, residents, and healthcare learners.
