Physicians Regional Medical Center - Pine Ridge is seeking an experienced RN Care Manager for the Emergency Room to support patient flow, determine admission status, and guide transitions of care.

This night-shift role requires BSN preferred and 1-3 years of acute care experience; you will collaborate with ED clinicians, hospitalists, care managers and social services to ensure smooth discharge planning and compliance with regulatory requirements.

Qualifications

• BSN preferred and active RN licensure in state of employment or compact state licensure.
• 1-3 years of experience in acute care or hospital setting required.
• 1-2 years in care management or utilization review preferred.

Responsibilities

• Assesses emergency department patients for admission necessity and correct status placement.
• Coordinantes with ED clinicians and social services for smooth transitions of care.
• Guides discharge planning and patient/family education on post-acute options.
• Ensures accurate documentation and regulatory compliance for transitions.
