Responsibilities

• Coordinates patient plan of care to ensure interventions are safe, timely and appropriate.
• Communicates clearly with patients, families, and health care team in person, by phone and in writing.
• Individualizes patient and family teaching based on a thorough assessment to meet their specific learning needs.
• Participates in evaluating the quality and effectiveness of nursing practice through quality improvement initiatives.
• Maintains professional knowledge in current nursing practice standards, laws and regulations.
