Saint Francis Hospital in Wilmington, DE is seeking a Pharmacist for the Emergency Room to provide direct patient care, medication histories, counseling and rapid response in emergencies. You will support trauma, stroke, sepsis and STEMI cases through coordination with the care team.

The role requires a Pharm.D. with hospital experience or a PGY-1 residency, Delaware licensure, and familiarity with Pyxis MedStation and EPIC. BLS/ACLS are required before orientation.

Qualificações

• Pharmacy degree (Pharm.D.) from an ACPE approved college of pharmacy required.
• ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of hospital experience is required.
• Registered pharmacist in Delaware required.
• Preferred: Experience with Pyxis MedStation, EPIC.
• BLS and ACLS certification before completion of orientation (will be provided if cannot provide upon hire)

Responsabilidades

• Assesses patient data to identify medication-related problems through profile reviews and pharmacy consults.
• Participates in medical emergencies and trauma, stroke, sepsis, STEMI as required.
• Reviews provider orders for drug appropriateness based on age, weight, and comorbidities; updates medication histories.
• Consults with providers and nurses on pharmacotherapy optimization to improve outcomes.
• Provides drug information and ensures safe use of medications.
• Documents therapy plans, monitoring, and follow-up in the EMR as per guidelines.
