Job summary

Qualifications

• Experience based on level (I, II, III, or IV).

Responsibilities

• Participates in performance improvement initiatives.
• Integrates evidence-based practice into clinical care.
• Conducts and documents accurate clinical assessments.
• Develops, implements, and evaluates the patient’s individualized plan of care.
• Uses nursing judgment to tailor care to patient needs.
• Manages patient assignments and coordinates with the care team.
• Assists with patient and family education and discharge planning.
• Precepts and mentors new staff as needed.
• Participates in departmental committees.
