Qualifications

• ACLS certification for acute care pharmacists.
• Active unrestricted Pharmacist license.

Responsibilities

• Provides direct patient care and medication management in emergency medicine.
• Collaborates with multidisciplinary teams to optimize drug therapy.
• Evaluates patient regimens, checks dosing and interactions, documents interventions.
• Monitors cost-effectiveness and outpatient transition of care from pharmacy services.
• Supports discharge planning and education for patients and staff.
