Formación

• Doctor of Pharmacy degree required.
• ASHP-accredited residency (PGY1/PGY2) with BPS board certification preferred.
• ACLS certification required for all acute care pharmacists.
• Current, full, active, and unrestricted license to practice as a clinical pharmacist.
• Recent hospital clinical experience (within the past 2 years).

Responsabilidades

• Provides direct patient care and medication management.
• Works collaboratively with multidisciplinary teams to optimize drug therapy.
• Clinically evaluates all patients daily with documentation of pharmacy recommendations and plans.
• Reviews drug regimens for indication, dosing, allergies, interactions, labs, and documentation.
• Provides cost-effective management of drug utilization and monitors outcomes.
• Educates BAMC staff and beneficiaries on pharmaceuticals as requested.
• Responds to Level I Trauma, code blue and massive transfusion emergencies.
• Performs discharge planning and smooth transition to alternate sites from a pharmacy perspective.
