Job summary

Physicians Regional Medical Center – Pine Ridge campus in Naples, FL is seeking a part-time RN Care Manager for the Emergency Room, nights (3 days per week, 8-hour shifts 3-11:30pm). The role focuses on assessing admission necessity, coordinating care transitions, and guiding patients and families to appropriate resources.

Qualifications include a BSN preferred and 1-3 years in acute care; 1-2 years in care management or utilization review are preferred.

• 1-3 years of experience in an acute care or hospital setting required.
• 1-2 years of experience in care management or utilization review preferred.

Responsibilities

• Assesses patients in the emergency department for medical necessity of admission and determines correct status.
• Coordinates with ED clinicians and community resources to avoid unnecessary admissions.
• Guides care transitions, including hospice or post-acute placement, and documents choices.
• Maintains accurate patient records and supports discharge planning with the team.
