Job summary

Rosaires Care is seeking an RN Care Manager for the Emergency Room to coordinate nursing care for high-acuity patients in a fast-paced, safety-net setting. The role involves comprehensive assessments and developing, implementing, and refining patient-centered care plans using evidence-based practices.

You will collaborate with physicians, social workers, and community partners, ensuring safe transitions of care and continuity of follow-up.

Qualifications

• Current RN licensure and strong emergency/acute care knowledge.
• Strong clinical assessment, care planning, and care coordination skills.
• Ability to manage complex cases and work effectively in a high-volume environment.
• Excellent communication, education, and advocacy skills.
• Team-oriented mindset with experience partnering across disciplines and services.
