Job summary

The Planned Parenthood of Delaware is seeking a Patient and Community Navigator (PCN) who provides equity-centered navigation bridging clinical care and community needs. You will support patients overdue for care, address social determinants, coordinate referrals, and facilitate warm handoffs across departments.

Responsibilities

• Provide equity-centered patient navigation to reduce barriers to care and bridge clinical services with community needs.
• Conduct proactive outreach to patients overdue for STI testing, contraception renewals, cancer screenings, and preventive care.
• Organize and manage timely appointments aligning with patient preferences and PPDE protocols.
• Identify, document, and resolve barriers to care including transportation, financial concerns, language access, and social determinants of health.
• Coordinate transportation and facilitate access to care with internal teams, vendors, and community organizations.
• Educate patients on care processes and follow-up expectations to improve completion and confidence.
• Maintain confidentiality and provide trauma-informed, nonjudgmental care for sexuality, reproductive health, and related topics.
• Contribute to quality improvement activities to reduce no-show rates and enhance patient experience.
• Collect program data and navigate metrics as directed by supervision; convene county-level community advisory groups.
