The Planned Parenthood of Delaware is seeking a Patient and Community Navigator to bridge clinical care and community needs. The role focuses on reducing barriers to sexual and reproductive health by supporting patients due or overdue for care, addressing social needs, and strengthening trust with communities we serve.

Formación

• High School diploma or equivalency; associate degree preferred.
• Minimum of one year in patient navigation or care coordination.
• Experience with electronic health records; Epic preferred.
• Experience with clinical appointment scheduling and patient outreach.
• Familiarity with sexual and reproductive health and social determinants of health.
• Community engagement or partnership-building strongly preferred.
• Proficiency in Microsoft Office; data tracking and documentation required.
• Bilingual or multilingual skills preferred (Spanish/English; Haitian-Creole/English).

Responsabilidades

• Proactively outreach to patients due or overdue for services using phone, text, and electronic communication.
• Organize and manage appointments to ensure timely care and patient preferences.
• Identify, document, and resolve barriers to care including transportation, finances, language access, and childcare.
• Coordinate transportation and logistics with internal teams, vendors, and community partners.
• Educate patients on care processes, preventive health, and follow-up expectations.
• Facilitate communication among patients, support persons, providers, and staff for continuity of care.
• Maintain confidentiality and provide nonjudgmental care coordination.
• Work with Medical Services to identify complex psychosocial needs and ensure warm handoffs.
• Participate in required training on trauma-informed care and equity-centered navigation.
